Project Microgrant Final Report Form

The Microgrant Final Report is due 30 days after the completion of the project. Final reports for FY25 are due no later than September 30, 2025.

Submission Instructions:

  • Upon submitting your report please *scroll down to confirm there were no errors in your application.
  • Upon successfully submitting you will receive an email confirming your submission.
  • If you do not receive the confirmation email within 12 hours, please contact us.
  • In order to save your partially completed form, please select Save and Continue at the bottom of the form.
  • When you select Save and Continue, the page will refresh and you will be given a unique link to return to your form. You must save the link by copying it or entering your email when prompted.

  • PART II: PROJECT INFORMATION

  • Please select the main project date (i.e. Opening Reception, Culmination Event, etc.). If there are multiple dates or a date range plese list those dates in the fields below.

  • Please include the number of social media posts used to promote the project.

  • What was not accomplished and why? What were the key lessons learned related to the project funded?
